Protect NoW helps reduce health inequalities and improve access to health and care services. It uses Population Health Management (PHM) and data to identify people who may need extra support.
How it started
Protect NoW began during the COVID-19 pandemic to support people most at risk. Working with partners across the system, we used data to:
- Identify people who may need support
- Contact them by text, letter or phone
- Help them access health and care services
Our approach
Protect NoW focuses on early action to keep people well. We:
- Use data to identify people at risk
- Focus on preventing illness or complications
- Target areas with poorer health outcomes
- Reach people who may not regularly access services
How it works
We use data from different sources to find groups of people who may benefit from support. We then contact them and:
- Offer advice and guidance
- Link them to relevant services
- Support them to improve their health and wellbeing
This helps improve access to care and ensures support reaches those who need it most.
Our impact
Since launching, Protect NoW has:
- Sent over 2.3 million communications
- Contacted more than 266,000 people
- Spoken with over 127,000 people
- Supported over 43,000 people into services
Looking ahead
Protect NoW continues to support people across Norfolk and Waveney, and will be expanded to support people in Suffolk in the future.
We will keep developing this approach to improve health, reduce inequalities and help people live healthier lives.
